PCMAC Application

Acceptance into the Primary Care Movers and Changers (PCMAC) is the result of a competitive process. PCMAC will accept approximately 45 fellows from Colorado’s urban and suburban areas for this one-year program. Applications must be complete to be considered and reviewed. Factors that weigh significantly on the final application score include: the quality of the essays and personal statement, applicant experience and readiness for the program (as demonstrated through a resume or CV), and the letters of recommendation. Fellows will also be selected to achieve a diverse cohort, representative of many types of practices, organizations and geographic areas.

Early April 2018 Notification regarding application outcome
May 4, 2018 at 6:00 PM Start of First Training Session (see the complete program schedule)
If selected, the applicant is expected to participate in the program.

Applications to Primary Care Movers and Changers (PCMAC) may be made through our online application portal.
  1. Please download and follow the PCMAC Application Instructions and Checklist.
  2. Please download the following three Application Signature Pages – to be completed and uploaded to your online application:
  1. Go to the online application portal to create a login (SIGNUP) and begin your online application.
  2. Complete and SUBMIT your online application.
